Therapeutic Case Manager, (Lyndon Town School)

NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 500 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. Responsibilities:

  • Meet with or have telephone contact with children and parents for evaluation, individual and family supportive interventions, crisis management and communication of significant information.
  • Responsible for the coordination of clinical treatment team meetings with students, parents and treatment team providers.
  • Responsible for the ongoing development and implementation of transition plans with referring agencies and/or school systems and potential post program placements.

QUALIFICATIONS

  • Bachelor’s in social work, psychology, human services, or a related field (Master’s preferred).
  • Minimum of 3 years of experience with emotionally/behaviorally challenged children and youth highly desirable.
  • Experience working with children and families in a therapeutic, school-based or case management capacity is a plus.

SALARY & BENEFITS

  • $25.29/hr plus, depending upon education and experience
