Job Description

The Nurse Practitioner works as part of the clinic provider team floating between our four locations and performing a variety of direct patient services including assessments, diagnosis, implementation of treatments, evaluations, and monitoring the general patient management plan for clinic patients. The Nurse Practitioner performs these functions in collaboration with a licensed physician in a primary health care setting.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

Obtains complete medical history from patient, and/or family, and/or previous medical record, etc.

Prescribes medications according to therapeutic goals, explains treatment to patients, and families.

Develops and assists patient and/or family therapeutic and educational plans to promote satisfactory patient outcomes.

Teaches and counsels individuals and families to assume responsibility for prevention of illness and for the promotion, maintenance and restoration of health.

Consults with physicians and other members of the health care team as necessary.

Performs other necessary duties as required by the health center to meet the goals of providing primary health care services.

Develops and explores cost effective means to provide safe, high quality, and accessible care.

Assesses, diagnosis, implements treatment, evaluates and monitors patients' health status.

Exercise sound clinical judgments based on assessment of the physical, psycho-social-cultural, spiritual and environmental needs of patients

Performs complete physical examinations on patients.

Orders routine and diagnostic lab and x-ray studies as indicated.

Diagnosis acute and chronic illness on the basis of clinical findings, laboratory and x-ray reports and/or the results of other diagnostic procedures according to approved protocols.

Formulates, initiates and monitors patient management plans for patients assigned to the Nurse Practitioner, using protocols and/or consultation with a supervising physician.

Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialist. Follows patients progress with physician.

Renders emergency care and treatments including cardiopulmonary resuscitation, starting IV fluids, and medications, sutures lacerations.

Performs routine procedures within scope of practice.

Maintains all patient records to comply with required standards, reviews records regularly and obtains physician counter signatures where necessary.

Attends all medical staff meetings and participates in the health center's QI/QA program.

Maintains effective working relationships with colleagues, counselors and others.

Requirements

1. Graduation from an accredited university, MSN.

2. Completion of accredited Nurse Practitioner program.

3. Current certification through a professional organization of their specialty.

4. Official recognition and current prescriptive authority through the state of South Carolina.

5. The ability to relate with effectiveness to patients and providers.

6. Must be qualified in Basic Life Support techniques.

7. Sufficient experience to carry out the duties of this position.
